Hospital Outpatient Visit

What to bring:

  • Name of procedure for which your child is scheduled
  • Name and telephone number of your referring physician
  • Insurance card(s)
  • Proof of legal guardianship or parent authorization, if you are not the parent
  • Social Security numbers, for you and your child
  • Referrals or authorizations from your child's primary care doctor (if applicable)
  • Medical records, such as X-rays or lab tests (if applicable)
  • A list of your child's medications
  • A stuffed animal, blanket or comfort item
  • Your child’s favorite book, toy, activity, etc.

Registration/Check-in

  • Directions to Sanford Children’s Hospital
  • Plan to arrive at the hospital 30 minutes before your scheduled appointment.
  • Valet parking is free and available at the main Sanford USD Medical Center entrance off 18th Street, between Lake Avenue and Grange Avenue.
  • Registration: A Greeter at the Information desk just inside the Locken Lobby will welcome you and give you directions to the Admissions area.
  • The Admissions area is also located in the Locken Lobby. While you register in Admissions, an Admission clerk will ask you some basic questions and place an identification band on your child.
  • A transporter will accompany you and your child to the Outpatient Procedure area. This is located in Trauma 5 (Emergency Department)
  • In the Outpatient Procedure area you will be greeted by a Pediatric Outpatient Registered Nurse or a Child Life Specialist. The nurse will complete a brief questionnaire on the computer about your child’s medical history and allergies, weigh your child, and measure your child’s blood pressure, temperature, heart rate and respiratory rate.
  • The Child Life Specialist will help prepare your child for the procedure and provide comfort and distraction during the procedure.

If your child will be receiving sedation for his/her procedure, a physician from the Pediatric Intensive Care Unit (PICU) will meet with you and examine your child before the procedure begins.
